At our institute we wish to offer you the best service possible. In order to be able to do this in the long term, we kindly ask you to observe the following: it can be necessary to make and adjust a new artificial eye once a year
- for members of health insurance companies upon presentation of a medical order (prescription, not a referral)
- upon presentation of a cost assumption declaration (authorisation) of another cost assumer, e.g. of a professional association
- for war invalids upon presentation of a cost assumption declaration by the relevant orthopaedic supply centre
